always call a provider's office in advance to make sure he/she is still
in a network. Network providers are independent contractors and not
employees or agents of National Benefit Builders Inc.. National Benefit Builders Inc. will in no way interfere
with or become part of the provider / patient relationship. Prices charged
by providers may be adjusted from time to time without notice to the
member. Occasionally, certain providers may offer products or services
to the general public at prices lower than our providers' prices. In such
an event, the member will be charged the lower price. Discounts on
professional services are not available where prohibited by law. This is
a discount card program and NOT INSURANCE and may be discontinued or
modified at anytime. DentAffordSelect is a brand name of National Benefit Builders, Inc..
National Benefit Builders, Inc. is not a licensed insurer, HMO, or any other
underwriter of healthcare services. National Benefit Builders, Inc. will not reimburse or otherwise
pay any portion of any provider's fee to a participating member. Providers
in the National Benefit Builders, Inc. network are solely responsible for the professional advise
and treatment rendered to members and National Benefit Builders, Inc. disclaims any liability with
respect to such matters. National Benefit Builders, Inc. reserves the right to replace any
network and will notify members accordingly.
